Program Guide for Service Providers

A resource for Service Providers that are participating or wish to participate in the Program. This guide explains the Program eligibility requirements, incentive levels, application process, and more. All application materials are included as attachments.

Fact Sheet for Commercial Customers

Application Materials

Applications for incentives must be completed and submitted by Participating Service Providers. A Service Provider may submit materials to join the Program before applying for an incentive or when submitting its first application for an incentive. All of the forms below are also included in the Program Guide for Service Providers.  

 

Submit these materials to become a Participating Service Provider:

     Service Provider Participation Agreement

      Certificates of insurance that meet the requirements specified in the Service Provider Participation Agreement

      W-9 Form (download from www.irs.gov)

  

Submit these materials to apply for a Solar Water Heating incentive:

      Customer Participation Agreement

      Project Application Form

    Diagram of the proposed System and list of all System components - Not required for OG-300 systems

    Digital photographs (if existing building) or site sketch (if new construction) showing collector location, orientation to due south, and shading, if any

      Digital photograph of existing electric water heater showing label with supplier name and model number - Only required for existing buildings

      Income Eligibility Form (Income Eligibility Form - Single Family or Income Eligibility Form - Multi-Family) - Only required for low-income residential incentive

      Service Provider bid with project estimate, purchase order, or letter of intent that includes itemized project costs - Only required for the low-income residential incentive

 

 Submit these materials once you have completed a project and wish to receive the incentive payment:

      Project Completion Form - (See Example Project Completion Form) - You will receive a pre-populated form from the Program with your Authorization to Install Letter

      Copy of the ten-year warranty on the collector, one-year warranty on installation labor, and any other warranties issued by the manufacturer(s), supplier, or installer

      Copies of all required state and local permits, such as the Plumbing Permit, Electrical Permit, and/or Residential Permit

      Digital photograph of collector(s) taken with the photographer’s back to due south

      Copy of the receipt(s) confirming amount and payment of all System(s) purchase and installation costs.  Receipts must include line item showing the incentive amount and must be signed by Customer

A resource for Service Providers, this guide provides detailed instructions on how to complete a Project Application. It also includes information on the additional supplementary materials (i.e. photos) Service Providers must submit with Project Applications.
